REHABILITATION SERVICES

Comprehensive Wound Care Management

Our Rehabilitation Services Include:

Patients referred to the Physical Medicine and Rehabilitation Service are evaluated for deconditioning and other rehabilitation needs.

Most of our patients fall into the following categories:

  • Post Surgical: includes Hip / Knee replacements, fracture management and multiple traumas. The needs for rehabilitation include close watch on the operation site and gradual upgrading of ambulation and weight bearing.
  • Post Medical Decoditioning: Includes supervision for respiratory and cardiac management, comprising Pulse Ox and BP check while they stand or exercise, check for balance and endurance.
  • Management of Deformities and Contractures: Involves prescribing splints, ROM Exercises and manual relaxants. 
  • Post Neurological Condition Management: Comprises stroke and head injury.
  • Pain Management: Executed through medical management or by injections.
  • Fall Prevention and Management: Includes evaluation for medical conditions, balance and muscle strength .

In coordination of physical therapist, speech therapist and occupational therapist rehabilitation plans are made weekly. Patient care conferences are attended as needed.

Our Wound Care Services Include:

  • Wound Care Consultation: Initiated on the request of a Primary Care Physician of the patient. Our team evaluates patient's records, examine all systems of the patient and skin wounds and make a comprehensive management plan.
  • Wound Debridement and Management of Infection: All our providers are trained wound specialits and new providers are provided three weeks of hands on training for wound debridement and taking tissue samples for culture.
  • Wound Care Education: We provide wound care education to the staff and development of a program in the Nursing Homes.
  • Work on avoiding or correction of IDPH tag, and talk to the inspectors if necessary.
  • Train on MDS 3.0 Documentation
  • Management of Dermatitis of Skin Lesions including Biopsy.
  • Management of Stoma Site such as Colostomy, Enterostomy, PEG Tube, Tracheostomy and Surgical Drain Sites etc.
  • Nutritional and other risk factor assessment and management.
